1. CLIENT INFORMATION CERTIFICATION By completing this agreement, I certify that all information I have provided is accurate, truthful, and belongs to me. Superior Transportation will rely on this information to schedule, coordinate, and complete my service. 2. TRIP DATE & SCHEDULE AGREEMENT I understand that my reservation is based on the Date of Service, Pick-Up Time, Scheduled Finish Time, and Scheduled Service Hours that I enter on this form. Scheduled Service Hours represent the number of hours I am contracting for. Any time beyond the scheduled hours will be billed as overtime at the hourly rate of the selected vehicle. Time is calculated garage-to-garage: from the moment the vehicle leaves Superior Transportation’s garage until it returns. 3. PICK-UP, DROP-OFF & VENUE ADDRESS ACCURACY I am responsible for providing correct pick-up, drop-off, and venue addresses. Incorrect or incomplete information may result in delays, route changes, or cancellation. Superior Transportation is not liable for delays caused by inaccurate addresses or last-minute changes. 4. MINIMUM HOUR REQUIREMENTS I understand that the following minimums apply: Sedans, SUVs, Passenger Vans: 2-hour minimum Sprinters, Limousines, Mini Buses, Party Buses, Charter Buses: 3-hour minimum Proms, New Year’s Eve, major events: 6-hour minimum Multi-day conventions / overnight trips: 12-hour minimum per day If my booking falls under these categories, I agree to the applicable minimum hours. 5. IDENTITY VERIFICATION REQUIREMENT For fraud prevention and reservation protection, I agree to upload: Front of my government-issued ID Back of my ID A selfie holding my ID My reservation cannot be processed without identity verification. 6. CREDIT CARD VERIFICATION (IMAGE UPLOADS) I agree to upload: Front of my credit card (last four digits visible only) Back of my credit card (CVV covered) These images are used only for identity and cardholder verification. Superior Transportation does not charge the card using photos. Payment will be processed only after an official invoice is sent. 7. CREDIT CARD AUTHORIZATION FOR ADDITIONAL CHARGES By signing this agreement, I authorize Superior Transportation to charge the card associated with my reservation for: Scheduled service hours Overtime beyond reserved hours Additional mileage or stops Late-night or extension requests Cleaning fees Damage fees No-show fees Last-minute cancellation fees Additional drivers (if required) Any overtime incurred will be automatically added to the final invoice. Attempting to dispute legitimate charges may result in additional fees. 8. PROCESSING FEE POLICY I understand that the processing fee (Standard, Premium, or Elite) is non-refundable. This fee covers administrative time, scheduling, route planning, availability checks, and fraud-prevention measures. My reservation is not in progress until the processing fee has been completed. ; 9. RESERVATION CONFIRMATION POLICY Completing this agreement alone does not confirm my reservation. Superior Transportation will review my information, verify identity, and confirm availability. My reservation is confirmed only after: I receive written confirmation from Superior Transportation, and I pay the official invoice sent after review. 10. CANCELLATION & NO-SHOW POLICY I understand that: Last-minute cancellations may be billed up to the full scheduled service amount No-shows are billed in full Failure to provide accurate locations or reliable contact information may be treated as a no-show 11. LIABILITY & SAFETY AGREEMENT Superior Transportation is not responsible for lost, stolen, or unattended items. Unsafe behavior, vandalism, or disorderly conduct may result in service termination without refund. All passengers must follow driver instructions for safety. 12. SIGNATURE & AUTHORIZATION By signing this form, I confirm that: I have read and understand this entire agreement. I agree to all contract terms and credit card authorization items. I agree to submit payment via invoice or payment request .I authorize Superior Transportation to bill any additional charges All submitted information is accurate and verifiable. 13. BINDING AGREEMENT & FRAUD PROTECTION By submitting this form, I acknowledge that this is a legally binding agreement between myself and Superior Transportation. This agreement remains binding unless: fraudulent, false, or misleading information is provided by customer, identity or payment method cannot be verified, the person submitting the form does not match the ID or cardholder. Superior Transportation reserves the right to refuse or cancel service if fraudulent or mismatched information is discovered. In such cases, the processing fee remains non-refundable. 14. OUTSTANDING BALANCES & COLLECTIONS POLICY Any unpaid balance resulting from this agreement — including scheduled service, overtime, damages, cleaning, no-show fees, or any additional charges — remains my responsibility. If payment is not made: Superior Transportation may submit the outstanding balance to a collections agency Collection fees, administrative fees, and legal fees may be added to the balance The debt may be reported to credit bureaus where applicable. 15. PAYMENT DEADLINES, CANCELLATION & COLLECTION FEES. I understand that all invoices, deposits, and payments must be submitted by the stated deadline. If I fail to submit payment by the deadline: My reservation may be automatically cancelled my scheduled vehicle and time slot may be released I may still be billed for the full cost of the reservation, including minimum hours and any applicable charges. × 16. PAYMENT REQUIREMENTS & FINANCIAL POLICIES Deposit Requirement A 50% deposit is required to officially confirm and secure the reservation. This deposit is non-refundable under all circumstances. Remaining Balance The remaining balance is due 20 business days before the scheduled service date. Failure to submit the remaining balance by the deadline may result in: automatic cancellation of the reservation, and being billed for the full cost of the reservation. Cancellation Policy: If I cancel within 20 business days of the scheduled event: I will be charged 50% of the total reservation amount, and any previously paid deposit will not be refunded. Refund Policy There are no refunds on deposits or payments for any reason. Late Payment Fees: Late payments are subject to a fee of: 2% of the outstanding balance, or $100 per month, whichever amount is greater. Returned Check / Chargeback Fee Any returned check or chargeback (disputed charge) results in a $500 fee, which will be added to the outstanding balance. If the remaining balance is unpaid after cancellation or after services are rendered, Superior Transportation may: submit the unpaid balance to collections add any collection fees, administrative fees, and legal fees to the amount owed.
